Job Description

  • The Care Manager - ED is responsible for coordinating patient care throughout the continuum of services, ensuring appropriate utilization of healthcare resources, facilitating discharge planning, and supporting quality patient outcomes. This role collaborates with interdisciplinary teams to assess patient needs, coordinate services, and ensure compliance with organizational and regulatory requirements.

Responsibilities

  • Coordinate patient care and services across the continuum of care.
  • Conduct utilization review activities to ensure appropriate use of healthcare resources.
  • Develop and implement discharge plans to support safe and effective transitions of care.
  • Collaborate with physicians, nurses, social workers, and other healthcare professionals.
  • Assess patient needs and identify barriers to care.
  • Facilitate communication among patients, families, and healthcare providers.
  • Maintain accurate documentation and patient records.
  • Monitor compliance with organizational policies and regulatory requirements.
  • Participate in quality improvement and patient care initiatives.

Requirements

  • Minimum of five (5) years of clinical experience as a Registered Nurse.

Experience with care coordination, utilization review, and discharge planning.

  • Computer literacy required.
  • PRI and Screen certification required at the time of submission.
  • Primary Source Verification of License and Registration of Registered Professional Nurse from the New York State Department of Education (NYSED) required.

Education: Bachelor's Degree in Nursing from an accredited nursing program required.

  • License: Current Registered Nurse (RN) license in the State of New York required.

Certifications

  • Basic Life Support (BLS) certification required.
  • PRI and Screen Certification required.
  • Primary Source Verification of License and Registration of Registered Professional
  • Nurse from NYSED required.
