- We are seeking an experienced Integrated Case Manager to coordinate patient care in an acute inpatient setting. This role focuses on discharge planning, care coordination, resource utilization, and ensuring continuity of care for patients with complex medical and social needs. The ideal candidate will have strong acute inpatient case management experience, excellent clinical judgment, and the ability to work collaboratively in a fast-paced healthcare environment.
Responsibilities
- Coordinate and manage patient care in an acute inpatient setting
- Lead discharge planning to ensure safe, timely transitions of care
- Collaborate with physicians, nurses, social workers, and external providers
- Assess patient needs and develop individualized care plans
- Arrange appropriate post-acute services including home health and community resources
- Ensure compliance with regulatory guidelines and case management standards
- Maintain accurate and timely documentation
- Advocate for patients and families to support optimal outcomes
- Address social determinants impacting recovery and discharge planning
- Participate in interdisciplinary care coordination meetings
Requirements
- Minimum 5+ years of case management experience
- At least 2+ years of recent acute inpatient case management experience required
- Strong discharge planning background required
Experience must be in acute inpatient settings (telephonic, utilization review only, or purely behavioral health backgrounds will not qualify)
- Ability to work independently with minimal training
- Strong communication, documentation, and interpersonal skills
- Excellent analytical and problem-solving abilities
- Proficiency with case management systems and documentation tools
Education
- Bachelor of Science in Nursing (BSN) OR
- Master of Social Work (MSW)
